-Be a resident or citizen of the USA
-Candidates must be 18 years of age or older
-Have a valid driver’s license (3 years driving history check required)
-Must possess a car with updated inspection, registration and insurance in candidates’ name
-Must successfully pass back-ground checks; FBI, PA state Police and Child Abuse clearances
-Requires 3 references to validate experiences; at least two must be professional references
-Successful drug testing required (random drug testing completed throughout term employment)
-Must possess a high school diploma and 3 years of experience working with people diagnosed with disabilities.
-An annual physical is required with a 2-step-TB, chest x-ray to show negative TB and a Tetanus vaccine given less than 10yrs

Choice Empowers complies with federal and state statutes and regulations that apply to the Waiver Programs and Services for which Choice Empowers is approved, licensed and qualified.
***Training, qualification and education are required per state regulations, per individual clients’ specific needs and at the discretion of Choice Empowers CEO for the safe and proper provision of services to the people who Choice Empowers support.
*Choice Empowers requires original validation documents to verify proof of qualifications. For positions requiring specialized qualifications, Choice Empowers will accept a copy of Bachelor’s Degree (official or unofficial transcripts are sufficient if graduation date is indicated), Copy of Diploma and Copy of Master's Degree if it indicates the field of study. If diploma does not indicate field of study, official or unofficial transcripts indicating major and coursework is required.
*In addition to the above requirements, Choice Empowers accepts reference letters as part of validating specialized qualifications. A request to contact references, past employers and school/college officials will be used to validate special qualifications. Resumes listing personal experiences as applicable for the position applied and any state documents that are acceptable by the governing departments will be required as proof.
